The Community Health Centers, Inc. (CHC) founded in 1979, is a 501(c) 3, tax-exempt corporation governed by a volunteer consumer controlled and community-based Board of Governors. The mission of Community Health Centers is to provide quality patient-centered primary healthcare to individuals regardless of their ability to pay. Our goal is for culturally relevant primary healthcare to be made available, assessable, affordable, appropriate, adequate and acceptable to all community members, particularly for individuals, families and groups who are vulnerable and underserved. CHC serves a medically underserved area (MUA) and health provider shortage areas (HPSA) for primary care, dental and mental health services. CHC is a safety net provider of primary health care to Salt Lake County's uninsured, low-income families. From simple roots as a "free clinic," CHC has grown to encompass four Salt Lake County-based health centers, two dental clinics and a rural farm worker clinic.
