The member organizations of Eldercare Partners offer a network of services across the Twin Cities metropolitan area. Geriatric care management and consulting and caregiver coaching, counseling, and training are offered by all of the member organizations. Members use the same definitions, policies, and procedures so that services are of a similar high quality. Fees for geriatric care management are set by Eldercare Partners. Caregiver Services are made possible in part by the Metropolitan Area Agency on Aging and the Minnesota Board on Aging through funding in the federal Older Americans Act. Voluntary contributions are requested to extend service to people with less means. Individually, the member organizations provide many other services, making Eldercare Partners a rich resource for transportation, home health care, chore and homemaking services, adult day programs, senior centers, respite care, caregiver education, and others (see member web sites).
