Apogee Outpatient Surgery Center provides the facility (operating room), nursing and technical staff, and the equipment, medication, and supplies necessary for your surgery. The bill from Apogee Outpatient Surgery Center is for these services only. The services provided by your surgeon, assistant at surgery, anesthesiologist, cardiologist, pathologist, laboratory, and durable medical goods supplier will be billed separately from our services. Please, direct any questions you have about those services to the appropriate provider¹s office. Our staff can assist you if you need to contact those providers. We will bill your insurance company only for the services rendered by Apogee Outpatient Surgery Center. To provide excellent service it is necessary that we have accurate information about you, your employer, and your insurance. We will ask you to complete a registration form and a health questionnaire. We will also need to take copies of your insurance card(s).
