Pueblo Pediatrics Ltd. is dedicated to protect your "non-public" Protected Health Information. This notice is to tell you how and why we collect that information, and who can access that information. HOW WE COLLECT YOUR CHILD'S INFORMATION: Your personal demographic information and that of your child: such as, your birth date social security number, and medical insurance information is obtained from you. That is why we ask you to complete our patient registration sheet, and why we ask for a copy of your insurance card. This ensures that the information we collect is correct. Each time you have a subsequent encounter in our office we will ask if the demographic and insurance information is correct. WHY WE COLLECT THIS INFORMATION:We collect this information so that we can treat your child's medical condition and obtain payment from you or your insurance. WHO HAS ACCESS TO YOUR CHILD'S PHI:Any person that you designate in writing, anyone directly involved in your child's medical care, people creating and maintaining your child's medical record, and those entities that need your child's information to process and submit health care claims in order to obtain payment for services. Entities such as Government Oversight agencies, Judicial and Administrative Proceedings, Law enforcement Agencies, Coroners and Medical Examiners, and Organ Procurement Organizations may obtain copies of your child's PHI. This practice has no jurisdiction over such entities. HOW WE PROTECT YOUR CHILD'S INFORMATION:Pueblo Pediatrics Ltd. will release your child's information only to those people who need the information in order to provide for the medical care and needs of your child. We maintain physical, electronic, and procedural safeguards to ensure that only those involved in your child's care have access to that information.
