Headwaters State Bank was originally located in Phelps, Wisconsin under the name Phelps State Bank. In 1955 permission was granted by the Wisconsin State Banking Commission to move the charter to Land O’ Lakes, Wisconsin and change the name to Headwaters State Bank.
