Home Health Care provides continuity of care in the transition from hospital to home, including medical treatments and disease management education. St. Joseph’s/Candler Home Health Care is the region’s only hospital-owned, Joint Commission accredited home health care service. We are a wholly-owned, non-profit, faith-based service.
