The goal is to prevent re-hospitalization and increase independence while recuperating at home. Our service include: Nursing, Case Management, Wound Care including Wound VAC treatment, rehabilitating therapies, Companion, Live-in Aide and Personal Care Assistant. Services are provided by: Registered Nurses, Licensed Practival Nursse, Physical, Occupational and Speech therapists. Medical Social Worker and Nurses Aides.
