Regular communication with the patient's physician and good patient/family education help make sure the treatment plan is effective.Community is a licensed and certified Home Health Agency, and Medicare/Medicaid-certified by the State of Texas. Our staff is well-trained and fully qualified to provide quality health care services in the home. All medical services are provided under the direction of the patient's physician.
