Wellstar now offers Patient Navigation Services - designed to assist patients with their referrals to specialty physicians and other system services. To make the referral process easier for the patient, Patient Navigation Services' staff will contact patients by phone and work with them to coordinate appointments for assessments, tests, consults and education. Staffed with Registered Nurses and Referral Specialists, the Service collaborates with physicians and other resources to reduce phone calls and wait times while insuring that the patient is well informed and educated. They also ensure that the test results and other information get back to the patient's primary care physician to be used in planning future patient care needs. Currently, Patient Navigation Services is focusing on supporting patients diagnosed with diabetes, or pre-diabetes. Our mission is to make sure that each and every patient gets easy access and support for all the recommended care and services needed to manage their disease well and prevent future complications. Our plan is to add other disease management support as patient needs are identified by patients, providers and/or staff.
