Home Health Care Partners is a not-for-profit agency licensed by New York State to provide home care services. Home Health Care Partners serves those persons whose care needs are not eligible for coverage under Medicare and Medicaid. Home Health Care Partners works collaboratively with physicians, health care facilities, senior living complexes and other home care agencies to enhance and provide home care services when needed. Services are available 24 hours a day. Home Health Care Partners provides both professional and paraprofessional services specifically, registered professional nurses, licensed practical nurses, home health aides, companions and sitters. Our staff is carefully screened prior to employment. Training as required by New York State Department of Health is provided by our nursing staff on a regular basis. Monthly educational programs are provided to help to assist home health aides to remain current with home care strategies and needs. Annual competency evaluations are held for each staff person in a clinical lab setting. Supervision of staff takes place in the home in a care setting. Registered professional nurses establish the care plan which provides direction for the staff providing care in the home. The nurse is available 24 hours a day to answer questions and provide direction to the patient/client, family and staff. Assessments may be scheduled at anytime based upon need.
